- Print and/or save a copy of each Official Entry Form and Studio Roster.
- Send ONE studio check or credit card (payment in full) to the address below by rebate* or closing date.

- Print Master Studio Worksheet, Official Entry Form, and Official Studio Roster.
- Complete one entry form per routine.
- Complete one Official Studio Roster and Master Studio Worksheet per studio.
- Send all entry forms together with Official Studio Roster, signed Master Studio worksheet, ONE studio check or credit card (payment in full) to address below by rebate* or closing date.

*IMPORTANT: All Entry Forms must be submitted together and be in excess of $1,000 with Master Studio Worksheet and signed liability waiver, Studio Roster, full payment, and postmarked by the rebate date to qualify for rebate. Incomplete entry packets are not eligible for rebates / discounts.
